
Overview
Tendonitis is a term commonly used to describe pain and irritation involving a tendon, although many persistent tendon problems are more accurately described as tendinopathy. A tendon is a strong band of connective tissue that connects muscle to bone and transfers muscular force to produce movement. Tendon problems can develop in the shoulder, elbow, wrist, hip, knee, ankle, and foot.
Tendon pain often develops when the tendon is exposed to more load than it can currently tolerate. This may happen after a sudden increase in exercise, repetitive occupational activity, changes in training volume, or repeated movements performed without adequate recovery. The condition can affect athletes and physically active people, but it is also common in people whose work involves repetitive lifting, gripping, reaching, or prolonged positioning.
Physical therapy is usually centered around progressive exercise rather than simply resting the tendon. The rehabilitation plan is adjusted according to the location of the tendon, irritability of symptoms, strength, functional limitations, and the demands placed on the tissue. Research across several common tendinopathies supports exercise-based rehabilitation as a central component of conservative management.
Anatomy and Biomechanics
Tendons connect muscles to bones and transmit force across joints. They contain predominantly collagen fibers arranged to withstand tensile loading while allowing efficient transfer of muscular force.
Healthy tendons continuously adapt to the loads placed on them. Regular, appropriately dosed loading can help maintain tendon capacity, whereas a sudden increase in mechanical demand may exceed the tendon’s current tolerance.
Common sites of tendon pain include:
- Rotator cuff tendons of the shoulder.
- Common extensor tendon at the outer elbow, often associated with lateral elbow tendinopathy.
- Common flexor tendons at the inner elbow.
- Patellar tendon below the kneecap.
- Achilles tendon at the back of the ankle.
- Gluteal tendons around the outer hip.
- Wrist and hand tendons exposed to repetitive gripping or movement.
The term “tendonitis” suggests inflammation, but persistent tendon pain is not always driven by inflammation alone. For this reason, clinicians often use the broader term “tendinopathy,” particularly when symptoms have been present for several weeks or months.
Signs and Symptoms
Symptoms vary according to the tendon involved and the activities that load it.
Common symptoms include:
- Localized pain over or around the affected tendon.
- Pain during or after activities that place load on the tendon.
- Tenderness when the tendon is pressed.
- Stiffness, particularly after periods of rest.
- Reduced muscle strength or endurance.
- Pain when performing repetitive movements.
- Reduced tolerance for sports, work, lifting, gripping, walking, or stairs.
- Symptoms that may initially improve during activity but return afterward.
- Swelling or thickening around some superficial tendons.
For example, Achilles tendinopathy may cause pain during running, jumping, or walking uphill. Patellar tendon problems may become noticeable during jumping, squatting, or climbing stairs, while rotator cuff-related tendon pain may become worse with reaching overhead or lifting the arm.
Symptoms should be interpreted in relation to the individual's activity and tendon capacity rather than by pain intensity alone.
Diagnosis
Clinical Assessment
Tendinopathy is primarily a clinical diagnosis. The therapist or physician will begin by asking about when the symptoms started, changes in physical activity, occupational demands, previous injuries, training history, and activities that increase or decrease symptoms.
The clinician will assess the location of tenderness, joint range of motion, muscle strength, functional capacity, movement patterns, and the tendon’s response to loading.
Specific loading tests may be used depending on the tendon involved. For example, calf raises may be assessed for Achilles tendon problems, resisted wrist extension for lateral elbow symptoms, and resisted shoulder movements for rotator cuff-related pain.
A useful assessment also considers the person's overall capacity. Someone who suddenly increases running from two days per week to six days per week may develop symptoms even though their running technique has not changed.
The aim is therefore not simply to identify a painful tendon but to determine why the tendon is struggling with its current workload.
Medical Testing
Imaging is not automatically necessary for every person with tendon pain.
Ultrasound can provide information about tendon structure and may be useful for selected superficial tendon conditions. MRI can provide more detailed information about tendon structure and surrounding tissues and may be considered when the diagnosis is uncertain, symptoms are persistent, or a significant structural injury is suspected.
Importantly, abnormalities seen on imaging do not always correspond directly with pain or disability. Clinical findings and functional limitations should therefore be considered alongside imaging rather than relying on a scan alone.
Current evidence also emphasizes that there is no single gold-standard diagnostic test for all tendinopathies. Diagnosis and treatment should be tailored to the tendon involved and the person's clinical presentation.
Complications and Risk Factors
Most people with uncomplicated tendinopathy can improve with appropriate load management and progressive rehabilitation. However, symptoms may persist when the tendon is repeatedly overloaded or when rehabilitation focuses only on short-term pain reduction.
Modifiable Risk Factors
Factors that may contribute to tendon problems include:
- Sudden increases in training volume or intensity.
- Repetitive occupational activities.
- Inadequate recovery between demanding sessions.
- Poorly planned return to sport after inactivity.
- Muscle weakness or reduced endurance.
- Limited joint mobility where relevant to the individual's movement demands.
- Repeated exposure to activities that exceed current tendon capacity.
- Continuing high tendon loads despite progressively worsening symptoms.
Exercise dose matters. Research examining resistance exercise for common tendinopathies suggests that appropriately prescribed external resistance can be beneficial, although the ideal intensity, volume, and frequency vary between individuals and tendon conditions.
Non-Modifiable Risk Factors
Age, previous tendon injury, individual tissue characteristics, and certain medical conditions may influence tendon health and recovery.
The tendon involved also matters. Achilles, patellar, rotator cuff, lateral elbow, and gluteal tendinopathies do not behave identically and should not all be treated using exactly the same exercise program.
Red Flags
Seek medical assessment promptly if you experience:
- A sudden pop followed by immediate weakness or loss of function.
- A visible change in the shape of a tendon or muscle.
- Inability to push off, walk, lift, or use the affected limb normally after an acute injury.
- Severe swelling or bruising following sudden trauma.
- Significant weakness that develops abruptly.
- Persistent symptoms that continue to worsen despite appropriate activity modification.
- Unexplained fever, redness, warmth, or systemic illness accompanying tendon pain.
- Numbness or other neurological symptoms associated with the painful area.
A sudden loss of tendon function can indicate a substantial tear or rupture rather than routine tendinopathy and requires appropriate medical assessment.
Physical Therapy Exercises
Exercise selection should be individualized according to the tendon involved, current strength, irritability, and functional demands. There is no single “best tendonitis exercise” that works for every tendon.
Progressive tendon loading is generally more important than repeatedly stretching or completely resting the affected area. Current evidence supports exercise therapy as a major component of conservative tendinopathy management, although the most effective exercise prescription can differ between tendon sites.
1. Isometric Tendon Loading
Choose a position that loads the affected tendon without producing excessive pain. Contract the relevant muscle and hold the position without allowing visible joint movement.
Begin with 4 to 5 repetitions of approximately 20 to 45 seconds, depending on the tendon and exercise selected.
For example, a person with patellar tendon pain may perform a supported isometric knee extension or Spanish squat, while someone with Achilles symptoms may use a calf raise hold.
Isometric exercise can be useful when dynamic loading is poorly tolerated, but it should not automatically replace progressive strengthening. Recent evidence suggests that isometric exercise may provide short-term symptom relief in some people, while its superiority over other strengthening approaches remains uncertain.
2. Slow Strengthening Exercise
Choose an exercise that loads the affected tendon through a controlled range of movement.
Perform the lifting and lowering phases slowly, using a resistance that allows good technique. A common starting point is 2 to 3 sets of 8 to 12 repetitions, with the exact dosage adjusted to the tendon and individual response.
Examples include:
Achilles tendon: controlled calf raises.
Patellar tendon: squats, leg press, or knee extension variations.
Lateral elbow: resisted wrist extension.
Rotator cuff: controlled resisted external rotation and elevation exercises.
As strength improves, gradually increase resistance rather than simply performing more and more repetitions.
Exercise therapy has been studied extensively across Achilles, patellar, rotator cuff, lateral elbow, and gluteal tendinopathies, with strengthening representing the most frequently investigated approach.
3. Eccentric Strengthening
Eccentric exercise involves controlling the lengthening phase of a muscle-tendon unit while it is under load.
For an Achilles tendon problem, for example, a person may rise onto the toes using both legs and then slowly lower the heel using the affected leg when appropriate.
Begin with 2 to 3 sets of 8 to 12 repetitions and progress gradually according to symptoms and strength.
Eccentric exercise has traditionally received substantial attention in tendon rehabilitation. However, current evidence does not support the idea that eccentric exercise is universally superior to every other strengthening method. A broader progressive loading program may be equally or more appropriate depending on the tendon and individual.
4. Heavy Slow Resistance
Once the tendon can tolerate basic strengthening, resistance can be increased while maintaining slow, controlled movement.
For example, a person with patellar tendon pain may progress from bodyweight squats to weighted squats or leg press. An individual with Achilles tendinopathy may progress from bodyweight calf raises to loaded calf raises.
A typical approach may involve 3 to 4 sets of 6 to 10 controlled repetitions, with sufficient recovery between sessions.
The goal is to gradually increase the tendon’s capacity to tolerate meaningful loads rather than keeping the exercise permanently easy.
5. Functional and Sport-Specific Loading
The final stages of rehabilitation should reflect the activities the person needs to perform.
A runner may progress from walking to jogging, running, hills, and faster running. An athlete may progress from strengthening to hopping, jumping, landing, acceleration, deceleration, and sport-specific changes of direction.
The progression should be based on capacity rather than a fixed calendar. Pain response during and after exercise, strength, function, and recovery between sessions should all be considered.
Exercise programs are generally well accepted by people with tendinopathy, but adherence and progression are important parts of successful rehabilitation.
Medical Treatment
Management depends on the tendon involved, duration of symptoms, severity, and functional limitations.
Activity modification is often useful, but complete rest is usually not the long-term solution. Instead, the aim is to temporarily reduce aggravating loads while maintaining appropriate movement and progressively rebuilding tendon capacity.
Pain-relieving medication may sometimes be recommended by a physician depending on the individual's medical history and the specific presentation.
Some patients may receive adjunctive treatments such as extracorporeal shockwave therapy or other physical modalities. Evidence for these treatments varies by tendon and condition, and they should generally complement rather than replace progressive rehabilitation.
Injection treatments require careful consideration. Some injections may provide short-term symptom relief but do not necessarily improve long-term tendon capacity. Corticosteroid injections, in particular, should not be considered a universal treatment for chronic tendinopathy because their longer-term benefits and potential risks vary by tendon and clinical situation.
Surgery is generally considered only for selected cases when substantial symptoms and functional limitations persist despite an appropriate period of conservative treatment, or when a significant tendon tear or rupture requires surgical management.
Finding a Physical Therapist
A physical therapist can determine which tendon is likely responsible for the symptoms and identify the activities or loading patterns contributing to the problem.
Good tendon rehabilitation should include more than massage or temporary pain relief. The program should gradually restore strength, tendon capacity, movement quality, and tolerance to the activities required at work, home, or during sport.
A useful rehabilitation program also teaches the patient how to manage training volume, recovery, symptom response, and progression so that improvements can continue after supervised treatment ends.
For athletes, the final stage should closely reproduce the demands of their sport before full return to unrestricted training or competition.
The goal is not simply to make the tendon hurt less. The goal is to improve its capacity to tolerate the loads that the person needs to perform safely and consistently.
